Submit supporting documents

Method 1:

If you ask someone to submit the proof of life documents on your behalf, the representative is required to submit all the following documents when going to provide the proof of life (For details, please refer to“Locations for providing the proof of life in person”):

1. The original of the proof of life documents that meet the requirements.  If you are unable to submit the original, a photocopy can be submitted but the original must be presented for verification (For details, please refer to the leaflet “Provide the Proof of Life to the Social Security Fund”);
2. Provide in writing the personal information of the beneficiary (including the name, Macao SAR Resident ID Card number, current address and contact phone number), and personal information of the representative (including the name and contact information).

Method 2:

If you submit the proof of life documents by mail, all the following documents must be mailed to the Social Security Fund (Address:  Macao P.O. Box No. 3094):

1. The original of the proof of life documents that meet the requirements (For details, please refer to the promotional leaflet “Provide the Proof of Life to the Social Security Fund”);
2. A photocopy of the beneficiary’s Macao SAR Resident ID Card;
3. Provide in writing the address and contact phone number of the beneficiary’s current residence, or the information about the contact person residing in Macao, if any (including the name and contact information).

NOTE: The proof of life documents must have the name, stamp and signature of the issuing entity, and the documents must prove that the beneficiary is still alive in the year in which the proof of life is provided, and must contain the beneficiary’s Macao SAR Resident ID Card information (including the name of the beneficiary, and the ID card number or the date of birth, and the information must be consistent with the information on the Macao SAR Resident ID Card).
