Service Target and Eligibility Criteria
Account owners who are not included in the list of fund allocation may file an objection statement to the Social Security Fund. If the account owner dies, his/her spouse, lineal relative by blood of the first degree of relationship or the heir may file an objection statement for him/her.
Formalities and Required Documents
1. Objection Statement–Declaration of Asserting the Right (CH/PT); (Sample)
2. Photocopy of the declarant’s identification document;
3. Photocopy of the account owner’s death certificate;
4. If the declarant is the deceased account owner’s spouse/lineal relative by blood of the first degree of relationship, a photocopy of the proof of relationship must be submitted;
5. In the case of the deceased account owner’s heir, a photocopy of the notarial certificate confirming the heir’s status or a photocopy of the judicial proof must be submitted (the original must be presented for verification).
