Service Target and Eligibility Criteria
If a Macao resident passed away in the year in which the disbursement was allocated, his/her spouse, family member of the first degree of lineal consanguinity or heir, as the declarant, can make a request on his/her behalf. The claimant must submit the following information for the claims procedure and relevant documents relating to the reason for the request of the deceased Macao resident in the previous calendar year (e.g. Statement of Request, declaration and relevant certification documents. Please refer to the Administrative Procedures Guide on the relevant grounds for details):
Formalities and Required Documents
1. Declaration of Entitlement(C/15) (CH/PT); (Sample of form completion EN)
2. Photocopy of identification document of the declarant of entitlement;
3. Photocopy of the death certificate of the deceased Macao resident;
4. If the declarant is the deceased Macao resident’s spouse or relative of first degree of lineal consanguinity, a photocopy of certification document to prove the relationship is required to be submitted;
5. If the declarant is the heir of the deceased Macao resident, a photocopy of the notarised certificate or judicial certificate to prove the declarant’s right of inheritance must be submitted. (If the applicant submits the document at the counter, a photocopy can be submitted and the original is required to be produced for verification; if the request is made by post, the original must be submitted.)
