If a resident of the Macao SAR passes away, their spouse, first-degree direct blood relative, or heir may file the request on their behalf.

1. The Statement of Request (RP) has been combined with the corresponding declaration form and updated. 
2. The Statement of Request (RP-02) has been updated to Statement of Request (RP-11). 
3. The Declaration Form (C/3) has been combined with the Statement of Request (RP) and updated to Statement of Request (RP-13) 
4. The Declaration Form (C/5) has been combined with the Statement of Request (RP) and updated to Statement of Request (RP-12) 
5. The Declaration Form (C/6) has been combined with the Statement of Request (RP) and updated to Statement of Request (RP-14) 
6. The Declaration Form (C/8) has been combined with the Statement of Request (RP) and updated to Statement of Request (RP-15)
