Formalities and Required Documents

1. To change personal identification information, such as the name or date of birth, recorded with the Social Security Fund, a beneficiary of the Social Security System or an account owner of the Non-Mandatory Central Provident Fund System (hereinafter referred to as the applicant) can go to a service point of the Social Security Fund and submit the following documents, either in person or through a representative:
   The applicant may submit the following documents, either in person or through a representative, to a service point of the Social Security Fund:
   1. A duly completed and signed Change of Personal Particulars Form (Download Forms CH / PT) by the applicant (Sample of form completion) (available at a service point of the Social Security Fund or can be downloaded from the Social Security Fund’s website);
   2. A photocopy of the applicant’s Macao SAR Resident ID Card;
   3. A photocopy of a document issued by the Identification Services Bureau about the applicant’s identification information change (the original document must be presented for verification).
2. To change information, such as the contact/correspondence address, phone number, or communication language, recorded with the Social Security Fund, a beneficiary of the Social Security System or an account owner of the Non-Mandatory Central Provident Fund System (hereinafter referred to as the applicant) may do so through the following methods:
   1. Electronically
      1. By changing the contact information with the Identification Services Bureau through its “Address Registration and Modification Service” via the “Macao One Account” mobile app or online platform, the Bureau is authorised to provide the relevant information to the Social Security Fund for information updates simultaneously.       2. Use the “Macao One Account” to log in to the “Online Services” on the Social Security Fund’s website. Website: https://eservice2.fss.gov.mo/Web
   2. In written form
      The applicant is required to complete and sign the Change of Personal Particulars Form (Sample of form completion) or provide in writing the applicant’s full name, the Macao SAR Resident ID Card number, detailed address, and contact phone number/mobile phone number for receiving mobile text messages, etc. The above documents must be signed by the applicant following the signature on his/her Macao SAR Resident ID Card, and a photocopy of the ID card must also be provided. The applicant may submit the required documents through any of the following methods:
      1. Email: at@fss.gov.mo;
      2. Fax: (853)2853 2840;
      3. By mail to the Social Security Fund (Address: Alameda Dr. Carlos d’Assumpção, N.º 249-263, Edf. China Civil Plaza, 18.º andar, Macau);
      4. Submit the required documents to a service point of the Social Security Fund, either in person or through a representative.
