Formalities of Filing an Statement of Request on Behalf of an Incapacitated Account Owner

Service Targets and Eligibility for Application
The account owner is an incapacitated person (e.g., in a coma, intellectually disabled, suffering from mental illness, dementia, or any condition that prevents them from managing their assets)

How to Apply
In person or through a representative.

With a Legal Agent
1. Statement of Request and relevant certification documents
2. Declaration for “formalities for assisting an individual to file a request”, signed by the legal representative (C/2)
3. A photocopy of the identity documents for both the legal agent and the account owner
4. A proof of legal agent status

No Legal Agent
A relative (limited to the account owner’s spouse and third-degree blood relatives, including parents/children, grandparents/grandchildren, siblings, great-grandparents/great-grandchildren, siblings of parents, nephews, and nieces);
1. Statement of Request and relevant certification documents
2. Declaration for “formalities for assisting an individual to file a request”, signed by the family member (C/2)
3. The relative must submit proof of their relationship with the account owner
4. A photocopy of the identity documents for both the account owner and the relative
5. A certificate issued by a public medical institution or social welfare agency confirming the account owner’s incapacity

Institution Providing Care for the Account Owner
1. Statement of Request and relevant certification documents
2. Declaration for “formalities for assisting an individual to file a request”, signed by the caregiver (C/2)
3. A photocopy of the identity documents for both the account owner and the representative of the institution providing care for the account owner
4. A certificate issued by a public medical institution or social welfare agency confirming the account owner’s incapacity.
