How to go through the formalities

Formalities and Required Documents

  1. To change the personal identity information such as the name or date of birth:
    The applicant may submit the following documents, either in person or through a representative, to a service point of the Social Security Fund:
    1. The completed “Change of Personal Particulars Form” (Download Forms CH / PT) signed by the applicant (Sample of form Completion EN) (The form is available at all service points of the Social Security Fund, or can be downloaded from the Social Security Fund’s website);
    2. A photocopy of Macao SAR Resident ID Card;
    3. A photocopy of a documentation issued by the Identification Services Bureau regarding the change of identity information (the original must be presented).
  2. To change the contact address or phone number:
    1. Electronically
      1. When entering the "Address Registration and Modification Service" of the Identification Services Bureau through the "Macao One Account" mobile app or "Macao One Account" online platform (https://www.mo.gov.mo/home) to change your contact information with the Bureau, the Identification Services Bureau is authorized to provide the relevant information to the Social Security Fund for information update at the same time;
      2. Log in to the Electronic Service Platform (https://eservice2.fss.gov.mo/Web) of the Social Security Fund’s website with your “Macao One Account” to apply.
    2. In written form
      The applicant is required to complete and sign the “Change of Personal Particulars Form” (Download Forms CH / PT) (Sample of form completion EN), or put in writing the applicant’s name, the Macao SAR Resident ID Card number, detailed address and contact phone number/mobile phone number for receiving text messages. The above documents must be signed by the applicant according to the signature on the ID card, and a photocopy of his/her Macao SAR Resident ID Card must be submitted together. The applicant may submit the required documents in any of the following ways:
      1. Email: at@fss.gov.mo;
      2. Fax: (853)2853 2840;
      3. By post to Social Security Fund (Address: Alameda Dr. Carlos d’Assumpção, N.º 249-263, Edf. China Civil Plaza, 18º andar, Macau);
      4. Submit the documents, either in person or through a representative, to a service point of Social Security Fund.
